Instructions

If you would like a physical copy of your 1095-C, please inform Human Resources via email hr@cardoneventures.com, via phone 631-944-2006 or via mail 16435 N Scottsdale Rd #440, Scottsdale, AZ 85254. Please include your full name, contact information, and the year of the 1095-C that you are requesting. A paper copy will be mailed to the address on file, within 30 days of receiving your request. Note: The option in UKG to receive a paper copy is not considered a request for a copy of this form, and you must notify HR separately.
