Requesting Form 1095-C Instructions

If you would like a physical copy of your 1095-C, please inform Human Resources via email hr@cardoneventures.com, via phone at 631-944-2006, or via mail at 16435 N Scottsdale Rd #440, Scottsdale, AZ 85254. Please include your full name, contact information, and the year of the 1095-C that you are requesting. A paper copy will be mailed to the address on file, within 30 days of receiving your request. Note: The option in UKG to receive a paper copy is not considered a request for a copy of this form, and you must notify HR separately. You may also reach out to Human Resources to grant paperless consent to email you the 1095-C if you have limited access to UKG.
