Employee Class: Regular Full-Time Eligible Employees

Employer pays: 100% for A $50,000 Life and AD&D Policy and 100% for the STD and LTD

Employee pays: 100% for Voluntary Life, Hospital Indemnity, Accident, and Critical Illness if the elect

Waiting Period: 1st of the month following 30 days from date of hire
